HoLEP vs Rezum

HoLEP and Rezum are two treatment options for benign prostatic enlargement. While both aim to relieve urinary symptoms, they differ significantly in how they work, how quickly they take effect, and the type of patient best suited to each approach. HoLEP involves the complete removal of prostate tissue, while Rezum relies on steam injections to gradually shrink the enlarged areas over time.

This article examines how these two BPH treatments work, their recovery processes, and which patients might be better suited to each approach based on their individual circumstances.

What is HoLEP Surgery?

HoLEP (Holmium Laser Enucleation of the Prostate) is a one-time surgical procedure that removes the enlarged portion of the prostate using laser energy. It is performed under general anaesthesia and provides immediate relief from urinary symptoms.

Complete tissue removal: A laser separates and removes all the prostate tissue causing blockage, similar to peeling the inside of an orange away from its skin.

Immediate symptom relief: Because the obstructive tissue is removed during enlarged prostate surgery, men often notice a quick improvement in urinary flow.

Single session treatment: The procedure is completed in one surgical visit, typically lasting one to three hours depending on the size of the prostate.

Effective for any prostate size: HoLEP is suitable even for very large prostates that may not respond well to other treatments.

What is Rezum Therapy?

Rezum is a non-surgical treatment for an enlarged prostate that uses controlled bursts of steam to reduce excess tissue. The steam causes the treated areas to shrink over time, easing urinary symptoms without removing the prostate tissue directly.

Minimally invasive procedure: A small device is inserted through the urethra to deliver steam precisely to the areas causing blockage.

Natural healing process: The steam-treated tissue gradually dies off and is absorbed by the body over three to six months.

Clinic-based treatment: Rezum is usually done under local anaesthesia and most patients return home the same day.

Tissue preservation: The structure of the prostate remains intact while the enlarged areas shrink naturally.

HoLEP vs Rezum: Key Differences

Below is a comparison across key areas for both the procedures to support informed decision-making:

1. Healing and Recovery Process

  • Hospital stay: HoLEP typically requires a one to two night stay. Rezum is usually performed in an outpatient setting with same-day discharge.
  • Catheter use: A catheter is generally used for two to four days after HoLEP. For Rezum, catheter use may last one to seven days depending on swelling and urine flow.
  • Symptom improvement: HoLEP offers noticeable relief within two to four weeks, as obstructive tissue is removed during surgery. Rezum results appear more gradually over three to six months as the treated tissue is absorbed by the body.
  • Activity restrictions: After HoLEP or Rezum, patients are generally advised to avoid strenuous activity and heavy lifting for several weeks to support proper healing.

2. Sexual Function Considerations

  • Ejaculation changes: Both procedures may affect ejaculation. Some men experience reduced or absent semen release. These changes are not harmful and do not affect the ability to orgasm. The extent varies based on anatomy and treatment type.
  • Erectile function: Most men maintain their ability to achieve erections after either procedure. Both are designed to avoid damaging the nerves responsible for erections.
  • Recovery timeline: Sexual function generally stabilises within six to twelve weeks following either procedure as inflammation resolves.

3. Patient Suitability

  • Prostate size: HoLEP is appropriate for prostates of any size, including those over 100 grams. Rezum may have practical limitations in larger prostates due to reduced treatment coverage.
  • Treatment goals: HoLEP is suitable for men seeking immediate and complete symptom relief. Rezum may appeal to those preferring a gradual, non-surgical approach.
  • Fertility and ejaculation concerns: Men with fertility goals should discuss potential ejaculatory changes with their urologist before choosing a treatment.

4. Treatment Durability and Follow-up

  • Long-term relief: Both treatments can provide symptom relief lasting ten years or more. HoLEP’s complete tissue removal may reduce the chance of tissue regrowth or retreatment.
  • Need for repeat procedures: Retreatment is uncommon with either option when performed correctly. Rezum may occasionally require a second session if symptoms persist.
  • Follow-up care: HoLEP often involves closer follow-up in the early healing phase. Both procedures require monitoring to track recovery and symptom improvement.

Making Your Treatment Decision

Choosing between HoLEP and Rezum depends on your prostate size, overall health, and treatment goals. HoLEP offers complete tissue removal and reliable symptom relief, making it a strong choice for men seeking a more definitive solution. A clear discussion with an experienced urologist will help ensure the treatment aligns with your recovery expectations and long-term priorities.
